Prior to colonization by denizens of Altair VI, Altair IV was home to a native civilization that was annihilated when a deadly virus gripped the planet.

The year 2270 saw construction finished on the Federation Central Hospital on this world. (ST roleplay module: All Our Yesterdays: The Time Travel Sourcebook)

Altair IV declared independence from Altair VI in 2267, which sparked a brief conflict ended by Federation peacekeeping forces. When a new president was inaugrated, Starfleet placed the greatest emphasis on showing support by assigning three Constitution-class starships to observe the proceedings. In 2285, the government on Altair VI continued to govern both worlds pending a resolution from the Federation Diplomatic Corps.

It's likely some type of resolution was attained by the 24th century.
