A simple [and not intricate, which is properly needed] search on the first page brought up by Google says James Dixon's site is the source. He sources that as both The Menagerie - which blatantly never gave it - and USS Enterprise Officer's Manual - which cannot be confirmed as the source by someone not possessing it [i.e. me].
