The Doctor was a serial killer. The Doctor was known for a series of bizarre incidents in which the victims were found to have had their organs harvested. He displayed an odd interest in only certain species. In 2255, he resurfaced and began a new series of attacks that caught the attention of James T. Kirk, Leonard McCoy and Nyota Uhura. The Doctor employed advanced technology in his attacks, including nanobots and analysis indicated that he may have originated in the Delta Quadrant.

Using advanced language recognition software, Cadet Uhura was able to deduce that The Doctor's unintelligible speech was mechanically garbled using an old Romulan military code from the era of the Earth-Romulan War. Furthermore, the actual garbled words were taken from the Romulan language, the Xanno language, and some from English.

When translated, his language revealed references to "5618", assimilation, and more. It should also be noted that the Borg designation for the Human species is Species 5618.
